Scholarships

The Connecticut Athletic Trainers' Association awards four undergraduate scholarships: the CATA Kathy Pirog Undergraduate Scholarship, the District 1 Cathy Horne Scholarship, the Carl Krein CATA Scholarship, and the CATA Penny F. Dunker-Polek Scholarship. The CATA offers these scholarships to athletic trainers who are enrolled full-time in a Connecticut college or university.  There are two additional sponsored undergraduate scholarships awarded each year: the CATA Select Physical Therapy Scholarship and the Elite Sports Medicine Mr. & Mrs. Paul A. Sirop Scholarship. Award recipients are chosen by the CATA Executive Council in conjunction with the CATA Research and Education Committee.

Note: The CATA Penny F. Dunker-Polek Scholarship is an undergraduate scholarship, and has a separate application form.

Common Application for: District 1 Cathy Horne Scholarship, CATA Kathy Pirog Undergraduate Scholarship, Carl Krein CATA Scholarship, CATA Select Physical Therapy Scholarship, and Elite Sports Medicine Mr. & Mrs. Paul A. Sirop Scholarship.

Eligibility Requirements for District 1 Cathy Horne Scholarship ($2,500), CATA Kathy Pirog Undergraduate Scholarship ($1,000), Carl Krein CATA Scholarship ($1,000), CATA Select Physical Therapy Scholarship ($500), and Elite Sports Medicine Mr. & Mrs. Paul A. Sirop Scholarship ($500):

1.  Application must be a full-time undergraduate student enrolled in an accredited Athletic Training Education Program in Connecticut.
2.  Applicant must be a student member of NATA and CATA.  Out of state students enrolled in an Athletic Training program in Connecticut should change their permanent address to their Connecticut mailing address on the NATA website.  This makes the student a member of the CATA and therefore eligible for the scholarships.  Student must also have a NPI number.
3.  Applicant must distinguish oneself academically, and as a participant in his/her athletic training program.
4.  Applicant may apply during his/her junior year, or immediately prior to his/her final undergraduate academic year (including 5th year).
5.  Applicant must confirm his/her intent to pursue the athletic training profession as his/her primary means of livelihood.

 

Nomination Instructions

Each certified athletic trainer may submit no more than one nomination for an undergraduate scholarship.

The following items must be submitted:

1.  The student application form complicated and signed by the nominee.
2.  A letter of recommendation to be completed and signed by a certified athletic trainer.
3.  Nomination from signed by the undergraduate Program Director or Department Head.
4.  Undergraduate official transcript.
5.  Copy of your NATA membership card.

The deadline for filing applications is April 1st.  Application packets must be postmarked no later than April 1st for consideration.

CATA Penny F. Dunker-Polek Scholarship Application

Eligibility Requirements for the CATA Penny F. Dunker-Polek Scholarship:

1.  Applicant must be a full-time undergraduate student enrolled in an accredited Athletic Training Program in Connecticut.
2.  Applicant must be a student member of NATA and CATA.  Out of state students enrolled in an Athletic Training Program in Connecticut should change their permanent address to their Connecticut mailing address on the NATA website.  This makes the student a member of the CATA and therefore eligible for the scholarship.
3.  Applicant must have academic standing as a junior or senior (including 5th year).
4.  Applicant must demonstrate an "above and beyond" attitude both as an athletic training student in the field and academically as well as other activities in which he or she may be involved.
5.  Applicant must have a demonstrated commitment to the profession.
6.  Applicant must be in good standing academically in accordance with the requirements of his/her Athletic Training Education Program.  There is no other minimum GPA requirement for this scholarship.
7.  Applicant must confirm his/her intent to pursue the athletic training profession as his/her primary means of livelihood.

Nomination Instructions:

The candidate must be nominated by a licensed athletic trainer in Connecticut.  The nominating athletic trainer may not submit more than one nomination for this award.

The following items must be submitted:

1.  A letter of recommendation from a licensed athletic trainer, on business letterhead.
2.  The student application form completed and signed by the nominee.
3.  One additional letter of recommendation from a licensed athletic trainer, faculty member, or clinical instructor on business letterhead.
4.  Copy of your NATA membership card.
5.  Affidavit of enrollment completed by the Athletic Training Education Program Director.

The deadline for filing applications for the CATA Penny F. Dunker-Polek Scholarship is April 1st.  Application packets must be postmarked no later than April 1st for consideration.
